What it does well
- Ketoconazole 2%: the treatment strength, significantly more effective than 1% on severe dandruff and seborrhoeic dermatitis in a head-to-head trial
- Licensed for dandruff, seborrhoeic dermatitis of the scalp and pityriasis versicolor (EU/UK pack)
- A short course: twice a week for 2–4 weeks, then once every 1–2 weeks to keep symptoms away
- Works on the scalp, not in the blood: no measurable plasma levels after use as a shampoo
- Pink, low-foam treatment shampoo; the 60 ml covers a 4-week course, the 100 ml a course plus a few months of maintenance
What it is not
- Not a hair loss treatment: it is licensed for dandruff, seborrhoeic dermatitis and pityriasis versicolor, nothing else
- Not a daily shampoo: more often than the leaflet says does not work better and dries the scalp
How to use it
Wet the hair, massage about a teaspoon into the scalp or the affected skin and leave it on for 3–5 minutes (your leaflet gives the exact time), then rinse well. Dandruff and seborrhoeic dermatitis: twice a week for 2–4 weeks, then once every 1–2 weeks. Pityriasis versicolor: once a day for up to 5 days. Conditioner goes on after Nizoral, not before. No improvement after 4 weeks? See a pharmacist or doctor.

What to expect
- Week 1: Itching and flaking often start to ease; in one trial the difference from a selenium shampoo showed by day 8.
- Weeks 2–4: Twice a week. Symptoms usually clear. No improvement after 4 weeks: speak to a pharmacist or doctor.
- After that: Once every 1–2 weeks. Dandruff and seborrhoeic dermatitis tend to return if you stop completely.
The honest part
- Not a hair loss treatment: Nizoral is licensed for dandruff, seborrhoeic dermatitis and pityriasis versicolor. The studies on ketoconazole and hair are small, old and partly funded by the manufacturer, and in the EU a medicine may not be advertised outside its licence. We make no hair claim for it.
- Minoxidil: the right order: Wash with Nizoral, rinse, let the scalp dry, then apply minoxidil to the dry scalp as usual. Washing after minoxidil rinses the dose away; washing before it does not.
- Hair can feel drier: Dry skin and changes in hair texture are uncommon side effects, and the formula carries 24% SLES. Use a conditioner after Nizoral and do not use it more often than the leaflet says.
